Since chiropractic benefits can differ from one plan to another—even among plans offered by the same insurer—new patients may find it helpful to confirm their coverage before the first appointment. Our fantastic staff at both our Greenville and Greer offices can answer most insurance questions by phone, so that there are no surprises once treatment has commenced.
What paperwork should new patients complete before their first appointment?
Simply Southern Chiropractic Center asks new patients to complete three forms before arriving for a first visit: the Informed Consent form, the Case History form, and the Laser Therapy Consent form. These forms are emailed to each new patient in advance. New patients can either fill them out prior to their new patient appointment or once they arrive. Completing this paperwork in advance shortens the check-in process and allows the care team to see the patient more promptly. Patients with questions about the forms can call either office or send a message through the website’s contact form.
